Palliative Care Outpatient Clinic

The Palliative Care Outpatient Clinic at the Zablocki VA Medical Center serves Veterans suffering from serious illnesses on an outpatient basis.  Team members include physicians, nurse practitioner, psychologist, a chaplain and social worker.  This interdisciplinary unit works to offer the best symptom management and informed consent process for Veterans with serious illnesses ranging from cancer to emphysema.

The clinic has scheduled appointment times to try meeting patient care needs.  The clinic team may be available for urgent consultations depending on timing and the nature of the care issue. 

If you feel you would benefit from an outpatient palliative care referral, ask your care providers for a consult for palliative care.
